This webpage contains all the resources in our enrollment process.

Medical

These forms must be filled out by a doctor and returned to school prior to the beginning of the school year.

  • Certificate of Child Health Examination (Physical Form)

    • A Certificate of Child Health Examination form must be completed for students entering grades:

      • Kindergarten

      • 6th Grade

      • 9th Grade

      • New from out of state

  • Proof of Dental Examination Form

    • A Dental Examination Form must be completed for students entering grades:

      • Kindergarten

      • 2nd Grade

      • 6th Grade

      • 9th Grade

  • Eye Examination Report

    • An Eye Examination Report must be completed for students entering grades:

      • Kindergarten

      • New to an Illinois School

  • Student Medication Authorization Form

    • A Student Medication Authorization Form must be filled out and returned to the school if your student needs to be given medication during the school day. A new form must be completed at the beginning of every school year.

Vaccinations

  • Kindergarten students are required to have a physical and up-to-date shots (4 year old boosters), an eye exam and a dental exam.

  •  6th graders require a 6th grade physical, a TDAP shot , a Meningococcal Conjugate Vaccine (due on or after their 11th birthday) , and their 3rd dose of Hepatitis B if they haven't already received it, and a dental exam.

  •  9th graders are required to have a physical and a dental exam.

  • 12th graders -Are required to have their 2nd Meningococcal Vaccine, (due on or after their 16th bday).
